RESUMO

La elección del momento más adecuado para realizar radioterapia en el tratamiento del cáncer de próstata es controversial ya que puede ser realizada inmediatamente posterior a la prostatectomía o como tratamiento de rescate ante una recaída. En este artículo, se realiza una búsqueda del tema, se seleccionan los ensayos clínicos con mayor evidencia y se analizan los resultados. Si bien existe beneficio en la radioterapia adyuvante, este resultado no se encuentra en todos los pacientes y sí se asocia a mayor toxicidad genitourinaria tardía, por lo tanto, la clave está en la selección del tratamiento según el paciente específico.


The choice of the most appropriate time to perform radiotherapy in the treatment of prostate cancer is controversial since it can be performed immediately after prostatectomy or as rescue treatment in case of relapse. In this article, a search for the topic is carried out, the clinical trials with the greatest evidence are selected and the results are analyzed. Although there is benefit in adjuvant radiotherapy, this result is not found in all patients and it is associated with greater late genitoutinary toxicity, therefore, the key is in the selection of treatment according to the specific patient.

PURPOSE: Latin America faces a shortage in radiation therapy (RT) units and qualified personnel for timely and high-quality treatment of patients with cancer. Investing in equitable and inclusive access to RT over the next decade would prevent thousands of deaths. Measuring the investment gap and payoff is necessary for stakeholder discussions and capacity planning efforts. METHODS AND MATERIALS: Data were collected from the International Atomic Energy Agency's Directory of Radiotherapy Centers, industry stakeholders, and individual surveys sent to national scientific societies. Nationwide data on available devices and personnel were compiled. The 10 most common cancers in 2020 with RT indication and their respective incidence rates were considered for gap calculations. The gross 2-year financial return on investment was calculated based on an average monthly salary across Latin America. A 10-year cost projection was calculated according to the estimated population dynamics for the period until 2030. RESULTS: Eleven countries were included in the study, accounting for 557,213,447 people in 2020 and 561 RT facilities. Approximately 1,065,684 new cancer cases were diagnosed, and a mean density of 768,469 (standard deviation ±392,778) people per available unit was found. By projecting the currently available treatment fractions to determine those required in 2030, it was found that 62.3% and 130.8% increases in external beam RT and brachytherapy units are needed from the baseline, respectively. An overall regional investment of approximately United States (US) $349,650,480 in 2020 would have covered the existing demand. An investment of US $872,889,949 will be necessary by 2030, with the expectation of a 2-year posttreatment gross return on investment of more than US $2.1 billion from patients treated in 2030 only. CONCLUSIONS: Investment in RT services is lagging in Latin America in terms of the population's needs. An accelerated outlay could save additional lives during the next decade, create a self-sustaining system, and reduce region-wide inequities in cancer care access. Cash flow analyses are warranted to tailor precise national-level intervention strategies.

Introducción: Actualmente existen discrepancias en cuanto a la indicación, dosis, técnica y contorneo de la sobreimpresión o aumento de dosis de irradiación sobre el lecho quirúrgico en el trata-miento adyuvante en cáncer de mama. Propósito de la revisión: El objetivo de la revisión es presentar la evidencia disponible en sobre-impresión del lecho quirúrgico en el tratamiento de cáncer de mama. Realizamos una revisión bibliográfica en MEDLINE pubmed, se analizaron 61 estudios publicados entre enero del 2000 a enero del 2022. Recientes hallazgos: La sobreimpresión sobre el lecho quirúrgico en la mama mejora el control local en aquellas pacientes con elementos de alto riesgo. Las diferentes técnicas disponibles son oncológicamente equivalentes. La delimitación del blanco de tratamiento debe guiarse por los clips quirúrgicos. Conclusiones: La indicación de dicho tratamiento deberá ser evaluado por los servicios de oncología radioterápica, definiendo sus protocolos y algoritmos de acción.


Introduction: Currently, there are discrepancies regarding the indication, dose, technique, and con-touring of the super impression or increase in irradiation dose on the surgical bed in adjuvant treatment in breast cancer. Purpose of the review: The objective is to present evidence on superimposing the surgical bed to treat breast cancer. We conducted a bibliographic review in MEDLINE PubMed; 61 studies published between January 2000 and January 2022 were analyzed. Recent findings: Superimpression of the surgical bed in the breast improves local control in patients with high-risk elements. The different techniques available are oncologically equivalent, and the surgical clips should guide the delineation of the treatment target. Conclusions: The indication of this treatment must be evaluated by radiotherapy oncology services, defining their protocols and action algorithms.

Resumen: Presentamos un caso de una paciente femenina de 27 años, con síndrome de Cushing ACTH dependiente con hipercortisolismo severo, causado por un macroadenoma hipofisario recurrente y resistente pese a dos cirugías transesfenoidales, radioterapia y terapia médica. Dada la falla en las diferentes terapias se realiza una adrenalectomía bilateral como tratamiento definitivo. La paciente fallece en el posoperatorio por causa no clara. Si bien la adrenalectomía bilateral ha sido reportada como un tratamiento efectivo en pacientes con enfermedad de Cushing, se ha relacionado con una mortalidad significativa vinculada con la severidad del hipercortisolismo y las comorbilidades presentes. En este caso la adrenalectomía izquierda se tuvo que convertir a cielo abierto, asociada con mayor morbimortalidad.


Abstract: The study presents the case of a 27-year-old female patient with adrenocorticotropic hormone (ACTH) dependent Cushing's disease and severe hypercortisolism caused by recurrent pituitary macroadenoma that was resistant to treatment despite two transsphenoidal surgeries, radiotherapy and medical treatment. Upon failure of the different therapies a bilateral adrenalectomy was performed as the final treatment. The patient died in after surgery although the case of death was not clear. Despite bilateral adrenalectomy having been reports as an effective treatment in patients with Cushing's disease, it has been related to significant mortality rates in connection with the severity of hypercortisolism and existing comorbilities. In this case the left adrenalectomy ended up being an open surgery, which is associated to a higher mortality rate.


Resumo: Apresentamos o caso de uma paciente de 27 anos com síndrome de Cushing ACTH-dependente com hipercortisolismo grave causado por macroadenoma hipofisário, recorrente e resistente, apesar de haver sido submetida a duas cirurgias transesfenoidal, radioterapia e terapia medicamentosa. Diante do fracasso das diferentes terapias, foi realizada adrenalectomia bilateral como tratamento definitivo. A paciente faleceu no pós-operatório por causa não esclarecida. Embora a adrenalectomia bilateral tenha sido relatada como tratamento eficaz em pacientes com doença de Cushing, ela tem sido associada a mortalidade significativa relacionada à gravidade do hipercortisolismo e às comorbidades presentes. Neste caso, a adrenalectomia esquerda teve que ser convertida para cirurgia aberta, associada a maior morbimortalidade.

Resumen: Los schwannomas vestibulares son tumores benignos de lento crecimiento. Aproximadamente el 90% son de presentación unilateral, el 10% restante, son bilaterales y se encuentran asociados al síndrome de Neurofibromatosis tipo 2. Las opciones de tratamiento son observación, cirugía o radiocirugía. La radiocirugía es un tratamiento seguro, con altas tasas de control local y baja morbilidad en schwannomas vestibulares menores a 3 cm de diámetro mayor. Reportamos la primera radiocirugía realizada en el sistema público uruguayo para este tipo de patologías.


Abstract: Vestibular schwannomas are slow-growing benign tumors. Approximately 90% are unilateral, the remaining 10% are bilateral and are associated with Neurofibromatosis type 2 syndrome. Treatment options are observation, surgery or radiosurgery. Radiosurgery is a safe treatment, with high local control rates and low morbidity in vestibular schwannomas smaller than 3 cm in major diameter. We report the first radiosurgery performed in the Uruguayan public system for this type of pathology.


Resumo: Os schwannomas vestibulares são tumores benignos de crescimento lento. Aproximadamente 90% são unilaterais, os restantes 10% são bilaterais e estão associados à síndrome de neurofibromatose tipo 2. As opções de tratamento são observação, cirurgia ou radiocirurgia. A radiocirurgia é um tratamento seguro, com altas taxas de controle local e baixa morbilidade em schwannomas vestibulares com menos de 3 cm de diâmetro maior. Relatamos a primeira radiocirurgia realizada no sistema público uruguaio pra este tipo de patologia.
